Lewis County food establishment inspection scores from February

Editor’s Note: These are reports from health inspections in Lewis County in February of this year. Lewis County Public Health’s inspections happen at various intervals, so reports from the health department include a few locations each. Red violations are those most likely to cause foodborne illness and must be corrected at the time of inspection. Blue violations relate to overall cleanliness and operational conditions and must be corrected by established deadlines or by the next routine inspection. Any establishment receiving 40 red points or any red point item repeated within an 18-month period is considered a high risk and must be reinspected. An establishment that receives 75 red points or 100 total points (red and blue) on a routine inspection or 40 red points on a repeat inspection will have their food establishment permit suspended.

Dragonfly, 1648 S. Gold St., Centralia — 10/10/20

The establishment did not have its food protection manager certificate available and was asked to procure it within 30 days. (5 red)

A food worker card was not current. The business was instructed to renew it within two days. (5 red)

Some foods were not properly marked. The establishment was referred to the food worker manual to learn to properly mark food with dates. (5 blue)

A deep cleaning of the facility due to food contact surfaces not being properly maintained was requested. (5 blue)

Inspection: Feb 5, 2026

Exit 59 Chevron/Subway, 416 Route 506, Toledo — 40/0/40

The holding temperature of corn dogs was reportedly 120 degrees, rather than the minimum required temperature of 135. (25 red)

No thermometer was present at the time of inspection. (5 red)

No food handler cards were present either. (5 red)

There was no active management over the establishment. (5 red)

Reinspection is required.

Ocean Sky, 601 National Ave, Chehalis — 8/5/13

The facility reportedly needed a deep clean, as it had a deep layer of grease and cheese built up. (5 blue)

The kitchen needed to be cleaned on contact surfaces. (3 blue)

All food handler’s cards expired. (5 red)
