Lewis County food establishment inspection scores for December and January

Editor’s Note: These are reports from health inspections in Lewis County between December 2025 and January of this year. Lewis County Public Health’s inspections happen at various intervals, so reports from the health department include a few locations each. Red violations are those most likely to cause foodborne illness and must be corrected at the time of inspection. Blue violations relate to overall cleanliness and operational conditions and must be corrected by established deadlines or by the next routine inspection. Any establishment receiving 40 red points or any red point item repeated within an 18-month period is considered a high risk and must be reinspected. An establishment that receives 75 red points or 100 total points (red and blue) on a routine inspection or 40 red points on a repeat inspection will have their food establishment permit suspended.

The establishment reportedly needed a deep clean. (5 blue)

The walk-in cooler was dripping water directly into cooling foods. (25 red)

Wire racks holding food were very dirty and potentially getting onto cold foods. Food held on the tray was discarded. (3 blue)

Cheese was stored in an ice bath but not adequately cooled. Ice needed to be available on all sides of the food. (5 red)

Rice in the warmer was extremely cold in the center. The container was below temperature and too big to keep the food hot. (25 red)

Dry aged meats and packaging were done without the required HACCP permit. (10 red)

Consumer advisory for uncooked eggs and other ingredients was not presented on the menu. (5 red)

Papa Bear’s Restaurant and Lounge, 207 W. Main St., Morton — 10/0/10

Deli meats in the walk-in freezer were dated from November or early December. Deli meets must be discarded or used after seven days. (10 red)

Two plates of shredded potatoes were expired. These plates were discarded. (5 red)

Cooking and cooling foods was not currently permitted at the establishment. The establishment was instructed to obtain a higher permit to continue cooking and cooling foods. (10 red)

Deli meats were observed sitting at 45 degrees fahrenheit, when deli meats must be preserved at 41 degrees or below. (5 red)
