Food Establishment Inspection Scores:From the Lewis County Public Health Department

Editor’s Note: These are reports from health inspections in Lewis County between December 2023 and March of this year. Lewis County Public Health’s inspections happen at various intervals, so reports from the health department include a few locations each. Red violations are those most likely to cause foodborne illness and must be corrected at the time of inspection. Blue violations relate to overall cleanliness and operational conditions and must be corrected by established deadlines or by the next routine inspection. Any establishment receiving 40 red points or any red point item repeated within an 18-month period is considered a high risk and must be reinspected. An establishment that receives 75 red points or 100 total points (red and blue) on a routine inspection or 40 red points on a repeat inspection will have their food establishment permit suspended.

Chevron, 12974 U.S. Highway 12, Packwood — 20/5/25

The mini-fridge was not maintaining cold holding, and a thermometer read 45 degrees fahrenheit. The location was instructed to turn down the mini-fridge and monitor it more closely. (5 red)

Raw meats were dripping onto cooked and cool foods storage, so the location was told to put the cooked and cool foods above the raw meats. (5 red)

The hot holding case was slightly below temperature, so it was told to be turned up and monitored more closely. (5 red)

Hands were not being washed in the dedicated sink, and the 3 compartment sink is for dishes only. (5 blue)

The active managerial control for policies and operations was not in place. (5 red)

The Blazer Bite, 615 W. Pearl St., Centralia — 5/0/5

Several temperature controlled foods were cold-holding at 43-45 degrees fahrenheit at the salad bar. These must be kept at 41 degrees or below at all times. (5 red)

The inspector needed to go over proper cleaning procedures with the business. (5 blue)
