A Lewis County Superior Court judge issued a $100,000 bench warrant on Wednesday for a woman accused of crashing into a Centralia home while under the influence of controlled substances in June 2023.
Monika Eva Levison, 35, of Tacoma, was charged in Lewis County Superior Court with one count of vehicular assault on May 19, 2025, after toxicology test results from the Washington State Patrol Toxicology Lab determined that there were drugs present in Levison’s system at the time of a crash on June 6, 2023.
According to court documents and previous Chronicle reporting, Levison allegedly fell asleep at the wheel, left the roadway while driving southbound on North Pearl Street, struck a power pole wire and then crashed her 2008 Nissan Altima into an occupied residence in the 2400 block of North Pearl Street in Centralia at approximately 2:47 p.m. on June 6, 2023.
The vehicle came to rest after hitting the victim, a 60-year-old woman who received “significant injuries to her leg” and was transported to Providence Centralia Hospital to be airlifted, according to the Lewis County Sheriff’s Office at the time.
She was ultimately airlifted to Harborview Medical Center in Seattle for medical treatment. Medical personnel reportedly determined that the victim sustained a fractured knee, back and pelvis, according to court documents.
A 4-year-old boy who was inside the house at the time of the crash was uninjured.
Deputies with the sheriff’s office, officers with the Centralia Police Department and first aid personnel were dispatched to the scene at 2:48 p.m., according to the sheriff’s office.
Officers who made contact with Levison reported that “her first words to them were ‘I’m clean’ and ‘I’m not impaired,’” according to charging documents.
When questioned, Levison allegedly said she had fallen asleep at the wheel and admitted to smoking marijuana the night prior and to using meth within the last three days.
At the time, then-Lewis County Sheriff’s Office Field Operations Chief Dusty Breen told The Chronicle, “The incident is still under investigation and it is anticipated she will be referred to the Lewis County Prosecutor’s Office for vehicular assault.”
Toxicology test results that were finalized in March 2025 reportedly indicated that Levison had
19ng/mL of THC, .30 mg/L of amphetamine and .10 mg/L of meth in her system at the time of the incident.
Levison was issued a summons notice on May 19 for a June 11 preliminary hearing in Lewis County Superior Court, which she was not present for.
A deputy prosecutor with the Lewis County Prosecutor’s Office confirmed in court Wednesday that the case was handled as a summons matter rather than an arrest due to the time it took to receive the toxicology test results.
