Food Establishment Inspection Scores: From the Lewis County Public Health Department

Editor’s Note: These are reports from health inspections in Lewis County between December 2023 and March of this year.

Lewis County Public Health’s inspections happen at various intervals, so reports from the health department include a few locations each.

Red violations are those most likely to cause foodborne illness and must be corrected at the time of inspection. Blue violations relate to overall cleanliness and operational conditions and must be corrected by established deadlines or by the next routine inspection.

Any establishment receiving 40 red points or any red point item repeated within an 18-month period is considered a high risk and must be reinspected. An establishment that receives 75 red points or 100 total points (red and blue) on a routine inspection or 40 red points on a repeat inspection will have their food establishment permit suspended.

Buckets were blocking the hand washing sink in the kitchen. This was corrected.

One food worker’s card was not approved in Washington state. (5 red)

Zabdi Teriyaki, 1621 Gold St., Centralia — 10/0/10

Cook was not knowledgeable of cooking temperatures for chicken. Chicken must be cooked to 165 degrees. (5 red)

One food worker’s card was not updated. All food worker cards must be current at all times. (5 red)

The business was asked to make sure a dedicated hand washing sink is clear and open at all times. The sanitizing solution on site was too strong.

Various foods were cold holding between 43 and 45 degrees. These must be kept at 41 degrees or below. The business was asked to use a digital thermometer to measure temperatures. (5 red)

Hand washing sink had mold growing in it and didn’t appear to be in use. Business employees were asked to clean their hands regularly when handling food. (10 red)

Sonic, 655 NW Arkansas Way, Chehalis — 25/5/30

Several food workers cards were not updated. The business was given two days to correct the issue. (5 red)

Several time-temperature control for safety foods were cold holding at 53 degrees. These foods must be held at 41 degrees or below at all times. The deli prep cooler air temperature was at 53 degrees. The food was placed in a walk-in cooler and/or placed in an ice bath until the temperatures were correct. (10 red)

The source of a leak in the walk-in cooler has not been fixed for a long time. This was reported to the health department. The business was asked to fix the issue immediately and then notify the health department. (10 red)

The business was asked to clean its ice machine thoroughly at least twice a week.

A pipe was leaking on the soda machine. The business was reminded all plumbing must be properly installed. (5 blue)

Peppermill Empress Inn, 1233 Alder St., Centralia — 5/0/5

The business was reminded they need a separate sink dedicated to washing hands.

A refrigerator was not maintaining safe temperatures. Food was cold holding at 45 degrees when it is required to be held at 41 degrees or lower.

Incorrect testing strips were in use.
