Milk storage area of the walk-in was not cold enough and milk was cold holding at 46 F. All potentially hazardous foods must be kept at 41 F or below at all times. Air temperature was adjusted. (10 red)

Ice machine needs to be cleaned. Please drain, clean and sanitize within one week. (5 blue)

Many potentially hazardous foods were cold holding between 42.2 and 52.4 F and must be kept at 41 F or below at all times. These were either discarded or moved to cool, based on time and temperature. Air temperature was adjusted. This is a repeat violation. (10 red)

Ladybug Espresso, Centralia 10/10/20

Hand wash sink was completely blocked with a stool in front of sink and a large container of powder in sink. Hand sink must be kept clear and accessible at all time. This was corrected. (10 red)

Dishes have not been washed, rinsed, sanitized and air dried as required. (5 blue)

There was no sanitizer in the bucket. This was corrected. (5 blue)

Because the approved plan for using the adjacent commissary is not being followed, a three-compartment sink must be installed in the stand. Basins must be large enough to accommodate your largest dish. Sink must be installed within 30 days. Obtain any required permits from the city and provide an indirect drain.

Many large tubs of shredded cheese had been prepped at 8 this morning and were still between 46 and 50 F. Please adjust procedures to ensure that cheese doesn’t increase above 41 F during prep or ensure it cools back down within four hours. These were uncovered and placed in the walk-in to cool. (10 red)

There was initially no sanitizer in buckets. When remade, solution was far too strong. Please dilute dispensed solution until corrected and then check frequently (at least every two hours). (5 blue)

The handwash station vessel does not have a free-flowing spigot. They were unable to find. An employee was designated to assist with handwashing. (10 red)

Establishment is operating without a permit. Continued operations require a permit application, permit feed, and fast track fees must be paid by Monday. (10 red)

There was no hand wash station set up. This is being corrected, but container does not have a continuous flow spigot. It is OK for this time, but please correct before any further events. This was corrected, except for the spigot. (10 red)

Hot dogs were not quite cold enough. These were placed back into the ice chest to cool to 41 F. Please monitor closely. (10 red)

Hand wash sink was blocked. Please keep the sink fully accessible and stocked with soap, paper towels and hot water (100-120 F) at all times. This was corrected. (10 red)

Lemons had not been washed and were still in the original packaging. The church kitchen is being used to wash lemons, which will then be placed in a food-grade container. (10 red)

Establishment is operating without a permit. Continued operations require a fast-track fee, permit feel and application fees, which are due by Monday. (10 red)

There is no hand-wash station. An ice chest has been converted into a hand-wash station. It has soap, paper towels and catch buckets. (10 red)

There was no one available with a food worker card. This was corrected. (5 red)

Tortas y Tacos Hernandez, Show Your Ride, Centralia 30/0/30

Hand wash station was completely blocked, both in front of and in sink. Hand sink must be completely accessible at all times and fully stocked with soap, paper towels and hot water (100-120 F). This was corrected. (10 red)

Head of lettuce had been washed, cut in half then stored in a plastic grocery bag. This is not allowed, due to the risk of contamination. Once cleaned, all produce must be stored in food grade containers. This will be rewashed before use. (10 red)

Two refrigerators were not holding temperatures quite cold enough and potentially hazardous foods were cold holding between 44.2 and 46 F. All PHFs must be kept at 41 F or below. Please adjust as needed. (10 red)

Establishment has a multiple event permit, but did not notify the Health Department of this event. Please note that the permit only covers events that the Health Department has been informed of. (10 red)

Hand wash station has only cold water and paper towels. Soap was still in the trailer and there were no buckets to catch wastewater and garbage. Please discontinue operations until corrected. (10 red)

There was initially no person in charge of the establishment. One worker found a food worker card and will be the person in charge. (5 red)

Red violations are those most likely to cause foodborne illness and must be corrected at the time of inspection. Blue violations relate to overall cleanliness and operational conditions and must be corrected by established deadlines or by the next routine inspection.

Any establishment receiving 40 red points or any red point item repeated within an 18-month period is considered a high risk and must be reinspected. An establishment that receives 75 red points or 100 total points (red and blue) on a routine inspection or 40 red points on a repeat inspection will have their food establishment permit suspended.
