Food Establishment Inspection Scores: From the Lewis County Public Health Department

Editor’s note: These figures are derived from inspections conducted by the Lewis County Public Health Department’s Food Safety Program.

 

Red violations are those most likely to cause foodborne illness and must be corrected at the time of inspection. Blue violations relate to overall cleanliness and operational conditions and must be corrected by established deadlines or by the next routine inspection.

Any establishment receiving 40 red points or any red point item repeated within an 18 month period is considered a high risk and must be reinspected. An establishment that receives 75 red points or 100 total points (red and blue) on a routine inspection or 40 red points on a repeat inspection will have their food establishment permit suspended.

American Legion, Centralia 10/0/10

There is a three-compartment sink in the bar that’s used for initial dish cleaning (first basin), ice/garnish dumping in strainer (second basin) and bleach water (third basin). Please rearrange your setup so that one basin is always kept clear and accessible at all times (completely empty). (10 red)

Chehalis Aerie (Eagles), Chehalis 45/0/45

Bare-hand contact was noted with ready-to-eat onions and lettuce when making hamburgers. Bare-hand contact is not allowed with any ready-to-eat foods. These were discarded. (25 red)

Cherry tomatoes were washed in a bowl in the dish sink containing dirty dishes and then in the sink designated only for thawing of raw chicken. Please designate the left basin of the three-compartment sink for produce washing and keep it clear at all times. Please clean and sanitize sink before washing produce. (10 red)

Potentially hazardous foods were cold holding between 45.7 and 53.8 F and must be kept at 41 F or below at all times. (10 red)

Due to the high risk violations, a reinspection is required. Please pay a $110 fee within 10 days.

The person in charge did not have a food worker card available. Please correct within one week. (5 red)

Starbucks, 1161 Harrison Ave., Centralia 5/10/15

Several food workers did not have valid food worker cards available. Please correct within one week and develop procedures to ensure that all workers maintain valid cards. (5 red)

Dishwasher is not sanitizing. Please correct by tomorrow. (5 blue)

Pitcher rinse sink still appears to be directly plumbed. Please correct when new nitrobrew coffee equipment is installed or as soon as feasible. This is a repeat violation. (5 blue)

Grandad’s Kettle Corn, Spring Youth Fair, Chehalis 10/0/10

Hand-wash station had cool water, which was also observed last week. Exemption permit is only valid if all food safety requirements are met. Strongly recommend replacing current setup with hose with a 5-gallon insulated container with continuous-flow spigot (currently operated by foot pump). (10 red)

Lind Concessions, Spring Youth Fair, Chehalis 30/0/30

Hand wash stations were set up outside but are required in the trailer. This was corrected. (10 red)

Apples were purchased ready to eat (please provide documentation Monday), but were then stored in a cardboard box. This is not allowed, and these apples must not be served until washed in running water (temporary setup available). (10 red)

Refrigerator was not cold enough and hot dogs were cold holding between 44 and 49 F. Refrigerator was adjusted. (10 red)

Tacos Before Vatos, 391 NW State Ave., Chehalis 15/0/15

Chicken had been cooked at home and was delivered at unsafe temperatures. This is not allowed. All foods must be prepared and stored in the mobile unit unless otherwise approved. This was discarded. (15 red)
