Food Establishments With Violations: Red/Blue/Total

Plaza Jalisco, Chehalis 35/0/35

Cooks need better working knowledge of cooking and hot-holding temperatures. Back-hand contact was noted with chips when placing in a bag for take-out order. These may not be served. Please ensure that bare hands are not used to serve or prepare ready-to-eat foods. (25 red)

Several potentially hazardous foods were cold holding between 43.7 and 54.7 F and must be kept at 41 F or below at all times, even when busy. Based on time and temperatures, these were all moved to other refrigerators to cool. Please ensure that all PHFs were 41 F or below when placed in the top of the prep cooler and do not overfill. This is a repeat violation, has been discussed many times, and must be reliably corrected. (10 red)

There were no test strips (160 F) available to test the dishwasher sanitization. Due to the third violation in the previous two years, a $100 fee is due to the Health Department within 10 days. If test strips have been obtained and just couldn’t be found, please provide documentation  (photo or receipt, for example) and the fee will be waived. (5 blue)

Indirect drain is still required on the three-compartment sink and plan of correction has not been submitted (due Nov. 22, 2016). Please submit a plan of correction within two weeks. Due to the third violation in the previous two years, a $100 fee is due within 10 days. (5 blue)

Wendy’s, Centralia 10/0/10

There are currently no plumbing fixtures at all in either restroom. A portable toilet is set up outside of the establishment and there are no hand washing facilities available. The establishment is required to discontinue all food service until plans are submitted to the Health Department, with the $200 review fee, detailing the scope of the remodel and how bathrooms (and least one) with hot running water (100-120 F) will be provided for employees. (10 red)
